Risedronate instruction
Risedronate is a pyridinyl bisphosphonate. The drug increases bone density and prevents further bone destruction. The effect of risedronic acid on bone metabolism is associated with inhibition of osteoclasts and reduction of bone resorption mediated by them. At the same time, the activity of osteoblasts and the process of bone tissue mineralization is preserved. It is used for osteoporosis, Paget's disease.
Risedronate side effects
Risedronate sodium may cause adverse reactions from various systems.
- Gastrointestinal disorders: constipation or diarrhea, various pains in the stomach, ulcerative manifestations of the stomach, esophagus or intestines, dysphagia
- Nervous system: headache, dizziness, asthenia, increased fatigue.
- Musculoskeletal system: myalgia, joint pain, cramps of the calf muscles.
- Senses: iritis (sclera hyperemia, pain in eyes), amblyopia, blurred vision, dry mucous membrane of eyes, ringing, and noise in ears.
- Skin: vasculitis, Stevens-Johnson syndrome and a toxic form of necrolysis.
- Allergic reactions: angioedema, urticaria, anaphylactic reaction.
If these reactions occur, tell your doctor, who will cancel or reduce the dosage of Risedronate.

Studies have shown that Alendronate increases bone density more than Risedronate after long-term therapy (12 months).
Contraindications
The drug is not prescribed in such cases:
- Hypersensitivity to risedronic acid and any of the components of the drug
- Hypocalcemia
- Severe renal failure (creatinine clearance less than 30 ml per minute).
- Pregnancy and breastfeeding
- Childhood under 18 years of age (effectiveness and safety are not established)
- Diseases related to the presence of ulcers and erosion on the mucosa of the digestive tract (gastric ulcer, erosive and atrophic gastritis, colitis, and enteritis of various etiologies). Pathologies of the esophagus (achalasia)
- Conditions in which it is not possible to be in an upright position for half an hour after taking the medication
- Malignant neoplasms
In rare cases, patients with hearing loss may have osteonecrosis of the external auditory canal. If the condition worsens, it is important to tell your doctor, who will advise on further use of Risedronate.
Sometimes there is osteonecrosis of the external auditory canal, especially in patients with hearing disorders, including those suffering from chronic infectious diseases of the ear.
How to take Risedronate?
Risedronate (Actonel) is taken orally. The tablet should be swallowed whole, without chewing, and with plain water (100-150 ml). Preferably, you should take the drug while standing. After taking the tablet, the patient should not lie down for 30 minutes.
The drug should be taken on the same day of the week. Absorption of risedronic acid depends on food intake, therefore Risedronate should be taken at least 30 minutes before the first meal, other medicinal product, or drink (except water). In adults, the drug is prescribed at a dose of 35 mg once a week.
If you miss taking Risedronate sodium, you should take the drug on the day when the patient remembers it. Then you should return to taking 1 tablet once a week on the day of regular intake. No 2 tablets should be taken on the same day.
During treatment, compliance with a diet sufficient in calcium and vitamin D is recommended. If necessary, additional prescription of calcium and vitamin D is possible.
The optimal duration of treatment of osteoporosis with bisphosphonates is not set. The need for continued therapy should be determined individually for each patient based on an assessment of the benefit-risk ratio of Risedronate, especially after 5 or more years of use.
No dose adjustment is required in elderly patients, because the bioavailability, distribution, and excretion rates in this group of patients (over 60 years of age) do not differ significantly from those in younger patients.
It is contraindicated in children and adolescents under 18 years old in view of insufficient data about its efficiency and safety.
Special Directions for Use
Before use, consult your physician and know all the particulars to be observed during treatment, among which are:

Beverages, products and medicines containing the minerals calcium, magnesium, aluminum and iron are incompatible with Risedronate.

Particular caution should be observed when using Risedronate Sodium in the following cases: in patients with a history of esophageal lesions that slow the passage of food through the esophagus or its emptying, such as stricture or achalasia; in patients who cannot remain upright for at least 30 minutes after taking the tablet; in patients with active or recently suffered from esophageal or upper GI lesions when using Risedronate.

Hypocalcemia should be treated before starting therapy with the drug. Other bone and mineral metabolism disorders (such as parathyroid gland dysfunction, hypovitaminosis D) should be treated simultaneously with Risedronate.

Risedronate should not be used during pregnancy and lactation.

Children. Because the data about the safety and effectiveness of Risedronate usage in children under 18 years old are insufficient, the drug is not used in pediatrics.

Influence of the drug on the reaction rate while driving vehicles and operating mechanisms. Although no direct effect of the drug on the reaction rate while driving vehicles or operating other mechanisms has been revealed. It should be considered that while taking Risedronate, adverse reactions on the visual organ may occur, which may be accompanied by visual acuity deterioration and photophobia.
